- Background: The program of Jorong Alert at District of Solok Province of Sumatera Barat is an activity of Community Based Health Program (CBHP) that involves health volunteers in reporting health problems of supervised families. The problem encountered in the program implementation is complexity of reporting format and understanding of the volunteers about thet format that brings difficulties for midwives in compiling reports causing delay in information delivery and non utilization of information. Objective: The study aimed to get an overview on the implementation of Jorong Alert information system from technical, organizational environment and behavioral factors and identify recording format relevant with needs of Jorong Alert stakeholders. Method: This qualitative research used explorative case study design involving 9 midwives, 43 volunteers/cadres, 18 staff of health centers, 7 community representatives and 2 staff of Solok District Health Office. Data were obtained through indepth interview and observation using interview guide complemented with recorder, camera and notebook. Sampling used stratified purposeful technique, analysis used coding and validity used triangulation. Result: Volunteers had major role in data collection
