  • Cerebral ischemia mostfrequently resultfrom occlusi' 'ethrombi thatformed in carotid vertebrobasilar or intracerebral arteries. Platelet activation is directly linked to cerebral arterial ischemia. therapydirected against the early platelet dependent steps of haemostasis rather than anti coagulant has proved useful in preventing episodes of cerebral ischemia onpatient with atherosclerosis. The anti platelet agents that are most used in the clinic include aspirin. dipyridamol. ticlopidine, clopidogrel, cilostazol. This agent inhibits platelet activation through different mechanism of action. Aspirin is thefirst line drug in the secondary stroke prevention. Combination of aspirin with dipyridamol produce synergistic anti thrombotic effect. Ticlopidine, clopidogrel are slightly more effective than aspirin in reducing the risk of ischemic event. Primarily cilostazol is used as treatment for peripheral arterial disease, recently it can befound as stroke therapy.Antiplatelet therapy with glycoprotein lIb/fila receptor inhibitor for acutestroke and as an adjunct to carotidarterystentingappearpromising. Recommendationfor using antiplatelet agent is noted in EUSf 2003, ASA 2006. WSC2006, PERDOSSf 2007.
