- Pregnancy In epileptic woman raises several concerns, Including the risk of more frequent seizures and the potential for aritieplleptlc drug-related teratogenicity. Every year, approximately 12,000 women in the United States become pregnant while taking anticonvulsant and around 6% of infants have malformations. How to treat pregnant women with epilepsy remains an enigma. Although anticonvulsantshaveteratogenicrisks,withdrawal of all anticonvulsantsbeforepregnancyis not a realistic option. Careful management Is essential to Increase the healthy outcome. The aim of the present study Is to review the general concepts of teratogenesis, as well as known genetic susceptibilities to teratogenic effects, with a special focus on the fist line antiepileptic drug such us carbamazepine, phenytoin and sodium valproate and the role of folic acid to prevent the malformation.. . Observational and experimental studies have shown, periconceptlonal folic acid supplementation can reduce the malformation. The American Public Health Service has recommended that all women who could become pregnant take 400~g/day, and other countries have adopted this recommendation. Unfortunately, evaluation of recommendation had showed no detectable Impact on incidence of malformation. Use of supplements should be followed by economic and educational lines. Intensive educational efforts and a very high proportion planned pregnancies seem to have been crucial factors. Actively promoting the use of supplements, public health agency and medical professional should consider Implementing the program.
