- ABSTRAK ABSTRACT Donggala district is the highest of malaria incidence (AMI) among districts in the Provmce of Central Sulawesi. Case finding has been carried out by mean of passive case detection (PCD) in the health centers. Diagnosis of malaria is carried out based on clinical symtoms which is weak in its validity and reliability. This study was aimed at developing guideline for malaria diagnosis based on clinical symstoms, improving microscopy in. the health centers in Donggala district. The study was done descriptively to obtain major symtoms among malaria cases attending the health centers at Biromaru, Dolo, Palolo, Banpres Tongoa, Kulawi and Donggala. A cross sectional study was carried out and information was obtained from laboratory technicians from the health centers. Data were analysed using single as well as combined symptom validity, bivariate and multivariate analysed and accomplished with additional scatter plot. The study observed 13 major symtoms from 591 cases. Ten of 13 symtoms were significantly prominent (p 1). Fever, cold and headache were the most prominent symtoms showing significant sensitivity, spesivity and overall associated with regression as (Y)= - 4.246 + 8.5587 fever + 8.9778 cold + 26.8654 headache. The results showed the high agreement between microscopy at the health centers and district laboratory of Palu. There were five symptoms which were closely associated to malaria, namely fever, cold. headache, chill and backache. Among those, only fever, cold and headache were siqnificantly associated to malaria in Donggala. . Keywords: clinic symptom - algorithm and blood stock
