- Background: The Health Office of the Special Territory of Yogyakarta Province has recommended a health financing model in accordance with the community condition of the province, namely social health insurance system (Jamkesos), and has received financial support from the Central Government to conduct a kind of tryout for the financing system change for the impoverished households (Gakin) in the Special Territory of Yogyakarta Province. From the budgetary requirement calculation, there was insufficient budget for conducting Jamkesos program and the respective District or Municipality was expected to cover the insufficiency. For this reason, the opinion of health stakeholders in Bantul District should be recognized toward the Jamkesos design of the Special Territory of Yogyakarta Province. Method: This is a non-experimental study with a cross-sectional design and consist of a combined quantitative and qualitative study to calculate the financial need for health of the Gakin and to identify the opinion of the health stakeholders, namely the Regent, the E Commission Chairperson of Local House of Representatives (DPRD), Health Office Leader, the Director of Local Public Hospital (RSUD), and the Kasubdin and Puskesmas (Community Health Center) Chairperson toward the Jamkesos design from the organizational, potential financial resources, obstacles and predicament points of view in its implementation. Results: The result of calculation of financial need of the Gakin by means of claim payment in Bantul Regency amounted to Rp6,570,627,325,00. per annum. The predicted fund allocation from the Central Government amounted to Rp 4,433,277,600,00. The contribution from the district for implementing Jamkesos program for the Gakin amounted to Rp 2,137,349,725.00. The opinion of the health stakeholders toward the Jamkesos program has been very positive, whereas the potential financial resources is from the District APBD (Local Expending and Revenue Budget), in addition to APBN (State Expending and Revenue Budget). For the initial phase Jamkesos will finance the poor people (Gakin), the Local Government through the APBD of Bantul District will cover the insufficiency. In addition, there is a potential obstacle that is related to the ownership and the status of Bape! Jamkesos. Some of the stakeholders were not agreed if the ownership of the Jamkessos is in Provincial Level, but suggest that it should be in district level. Keywords: opinion, stakeholder, Jamkesos
