- Background: Surya Husadha Hospital is a private hospital located in Denpasar. Since 1992 Surya Husadha hospital has implemented managed health care so called as Surya Husadha Hospital Club (SHHC). SHHC has about 15.000 members. Data showed that SHHC was suffering loss. This study aimed to explore the loss of SHHC, factors that cause the loss. and ways to solve the problem. Methods: This is an evaluative case study which used qualitative and quantitative approaches. SHHC of Grand Hyatt Hotel was the unit of analysis. Secondary data were collected from financial department and medical record department. Results: The study showed that SHHC of Grand Hyatt Hotel lost Rp 137.669.144.5.- in a year. Factors that caused the loss included: (I) fee for service as method of payment for provider: (2) predominant curative services: (3) gatekeeper functioning ineffectively (4) unavailable pharmaceutical formulation
