- Vascular dementia (VaD}, the second most common dementia after Alzheimer disease AD), has great potential for prevention and treatment. VaD relates to different vascular mechanism and changes in the brain and has different causes and clinical manisfestations, reflecting complex interactions between vascular etiologies, changes in the brain, host factors and cognition. As a consequence of the variation in definition of VaD, different diagnostic criteria have been proposed. Among these eight operational criteria, only three have been used in epidemiological research. Although comparisons between epidemiological studies remain impeded by methodological differences, they all suggest that stroke increases the risk of VaD. Depending upon composition of cohorts, prevalence rates of post stroke dementia varied between 13.6% and 31.8% at 3 months. The incidence of VaD is 6 per 1,000 persons per year in subject 75 years and older. One-fourth of stroke patients develop new-onset dementia one year after stroke. The relative risk of new onset dementia within four years is 5.5. The cumulative incidence of dementia in the stroke cohort increased from 7% at year 1 to 48% at year 25. Identification and treatment of cerebrovascular risk factors is logical from a public health standpoint. In this regard there are both mid life factors such as hypertension, cigarette smoking, alcohol comsuption and dietary factors, and later life, such as hypertension and atrial fibrillation, that may have a high enough population-attributable risk to be identified as primary targets of prevention for VaD. Several therapeutic trials have been performed up to now in VaD, with drugs differing in type and mechanism of action. Key words : vascular dementia - diagnosis criteria - prevention - treatment
