The high-risk detection during pregnancy is purposed to find the risk factors of fetal and maternal complications on delivering. The prognosis of disease will be better if the risk factors of the disease can be earlier detected before the clinical signs of the disease appear. Therefore, it could decrease impairments, complications, and the risk of death. The objective of this research was to know inpact of complexity of implementation in antenatal care by midwife toward delivery status. This research utilized an observational and case control design. The participants in this research were all mothers who delivered on 1 January 2001 to 28 February 2002 at the General Regency Hospital of Purworejo with inclusion and exclusion criteria. Sixty (n= 60) participants (case= 30 and control= 30) participated in this research. Bivariat analysis showed a correlation between high risk and the incidence of delivery status was significant statistically with CI 95 percent and alpha 0.05 (p= 0,01 OR=6,50CI95percent= 1,57-29,04). Theexternal factors that was significant statistically were education and paritas. Logistic regression analysis showed a correlation between high risk detection and the incidence of prolong labour was not significant statistically (p= 0,06 OR= 8,80 CI 95 percent= 0,87-90,00). Epidemiologycally or clinically, high risk risk detection could increase the incidence of prolong labour. It was concluded that bad complexion of implementation of high risk detection in antenatal care by midwife in incrised the risk of prolong labour, after it was influenced by education and parity factors.
