- Chronic periodontitis is defined as the inflammation of tooth supporting tissues caused by periodontopathic bacterial infection. The focus of periodontal therapy is to eliminate bacteria through scaling and root planing (SRP). The pathogenesis of chronic periodontitis is influenced by host inflammatory response. Bacterial infection causes proinflammatory cytokines secretion. When cytokines are released in large amount and long period of time, they induce periodontal loss of attachment and alveolar bone resorption. The involvement of host inflammatory response encourages the development of adjunctive therapy to SRP by applying therapeutic substances into periodontal pocket. The purpose of this adjunctive therapy is to modify host inflammatory response which will increase the effectiveness of SRP. One of the substances developed for this purpose is Aloe vera, which possesses the ability to decrease Prostaglandin E2 secretion due to carboxypeptidase and aloe emodin contained in the plant The subjects of this research were chronic periodontitis patients with periodontal pocket depth of 3-5 mm. The subjects were divided into 2 groups: test and control groups. There were 14 subjects in each group. The subjects in test group were treated by SRP and Aloe vera gel application while in control group the subjects were treated by SRP alone. Prostaglandin E2 data were retrieved on day 0, day 10, and day 30. Data was then analyzed using Kruskal Wallis test with 95 % significance. Kruskal Wallis test results showed a decrease of Prostaglandin E2 in both groups with a significant difference between test group and control group (p<0,05). The conclusion of this study was the application of Aloe vera gel as an adjunctive therapy to scaling and root planing was able to decrease gingival crevicular fluid Prostaglandin E2.
