There are many factors that contribute mortality in patients with severe traumatic brain injury, one of them is hyponatremia. The study of hyponatremia as a predictor of mortality in patient with severe traumatic brain injury in Indonesia has not been done yet. The objective of this study is to assess hyponatremia as a predictor of mortality in patients with severe traumatic brain injury. The method of this study was casus control. Data were collected from medical records of patients hospitalized in Sardjito Hospital Yogyakarta from Oktober, 1st 2012 to Oktober, 31st 2013, with code of diagnosis were S06.4, S06.5, S06.6, S06.7 dan S06.8. We analysed the data using chi-square test in bivariate analysis followed by logistic regression in multivariate analysis. Total subjects that match to the inclusion and exclusion criteria were 102 patients. Subjects were divided into 2 groups: 51 subjects as casus group and 51 subjects as control group. There were 15 subjects hyponatremia in casus group and 51 subjects non hyponatremia in control group. Hyponatremia was statistically significant as a predictor of mortality in patients with severe traumatic brain injury (p=0,03) with OR 3,909 (95% CI 1,031-14,827) in bivariate analysis. The other variables that also statistically significant as a predictor of mortality in patients with severe traumatic brain injury were Glasgow Coma Scale (GCS) score, leukositosis, hypoalbuminemia, hyperglycemia, presence of intracranial lesion. Hyponatremia was an independent predictor of mortality in patients with severe traumatic brain injury (p=0,03), with OR 8,956 (95% CI 1,230-65,202) in multivariate analysis. The other variables that also statistically significant as an independent predictor of mortality in patients with severe traumatic brain injury were GCS score, leukositosis and presence intracranial lesion. As the conclusion of this study, hyponatremia is an independent predictor of mortality in patients with severe traumatic brain injury.
