  • Background: Timing for tracheostomy in post craniotomy patient is important. Early tracheostomy was suspicious better than late. Objektive: To compare outcome of ultraearly tracheostomy and non ultra early tracheostomy. Method: This retrospective study compare outcome of Blood Gas Analysis, Length of Stay in Intensive Care Unit and mortality in post craniotomy patient, between ultraearly and non ultra early tracheostomy using data from the medical records in Sardjito General Hospital, Panti Rapih Hospital and Bethesda Hospital from January 2008 to December 2013. The outcome analyzed using independent-t test and dependent t test with level of significance p<0.05. Correlation between length of hospitalization and mortality were analyzed using regresi logistic. Each variable estimated by Relative Risk. Results: PCO2 and pO2 were identified to have a significant difference in the outcome of pre- and post- in ultra early tracheostomy. pO2 have a significant difference in the outcome of pre- and post- in non ultra early tracheostomy. There were no statistically significant difference length of hospitalization and mortality between ultra early tracheostomy, and all variabels in non ultra early tracheostomy. Conclusions: PCO2 and pO3 were advantages of ultra early tracheostomy. Keywords: severe head injury, craniotomy, ultra early tracheostomy, blood gas analysis, length of stay ICU, mortality
