- The state of hypoalbuminemia is one of risk factor for infectious complications in patients with acute stroke through its role lowers the immunity. Urinary tract infection is known as one of the complications of stroke. The aim of this study was to determine the role of hypoalbuminemia on the urinary tract infections event in the acute phase of stroke patients. The design of study was case control. Subjects were acute phase of stroke patients and divided into two groups consisting of 30 patients who were diagnosed with urinary tract infection and 30 patients who were not diagnosed with a urinary tract infection. Serum albumin levels and other risk factors at the time of initial hospital admission obtained from patient's medical record. Subjects consisted of 37 men (61.7%) and 23 women (38.3%) with mean age 65,73 years. Hypoalbuminemia was found in 26 (43.3%) subjects. Variables that had significance and risk factor for the incidence of urinary tract infections in patients with acute stroke were hypoalbuminemia, female gender, leukocytosis and coma. Multivariate analysis showed that albumin is an independent risk factor and had very significant relationship to the incidence of urinary tract infections in patients with acute stroke (OR=34.72, 95% CI 5.77-208.81, p=<0.001). Hypoalbuminemia, female gender, and leukocytosis was a risk factor for urinary tract infections in patients with acute stroke. Apropriate management of these conditions can reduce the likelihood of complications such as urinary tract infections in the acute phase of stroke patients
