  • Background: The hospital became a Local Public Service Agency (BLUD) is a necessity. The ease of the BLUD hospital including the financial flexibility to implement a healthy business practices. In the middle of the competition and the external environment changes, the BLUD shape is an opportunity to enhance service. To be BLUD, stakeholder support is required from both external and internal. Sumbawa Public Hospital (RSUD Sumbawa) Stakeholders support is reasonably low, that require analysis of stakeholders. Objective: Describe the RSUD Sumbawa stakeholders support to apply BLUD and propose solutions. Research methods: A case study, to answer how RSUD Sumbawa stakeholders support is, with a Schmeer (1999) stakeholder analysis. Stakeholders are, internal stakeholders: the Director and staff of RSUD Sumbawa and external stakeholders: local governments and legislators. The course of the research interviewing, to obtain information about power , leadership, knowledge, position, interests and alliances. Results: 14 key stakeholders has been identified. The level of knowledge, 13 people low, 5 people moderate and 5 people high. The position stakeholder support, except 3 internal stakeholders from functional staf. Stakeholders see the benefits, improved services, more independent and hopes to improve the welfare of employees. There are three groups of potential stakeholders to form an alliance. Conclusions and recommendations: The position of the external and internal stakeholders largely supportive, except 3 functional hospital staff who declared themselves do not support. R1 as the most important stakeholders of 14 key stakeholders. Low level of knowledge has no direct effect making stakeholders do not support. There are still weakness in internal RSUD Sumbawa and need a shared commitment to prepare for implementing BLUD. Hospital leader need to invite stakeholders to discussion, making a commitment using a professional facilitator or with specific strategies aimed at maintaining the support and cange not supported stakeholders to be supportive
