- Background: at the Hospital General Dr. Sardjito Yogyakarta Center on the part of the distribution, in the provision and delivery of medical record file to get to the clinics are experiencing delays. Files that are not distributed in accordance with the standards of the time of the shipment i.e. & amp �30 minutes. In November 2013 Dr. Sardjito Yogyakarta was change the file distribution system of medical record. Distributing medical record previously done by manual, i.e. distribution officers carry files medical record of patients from filing to clinics by using a trolly and also lift patients, now distributing medical record is already using the freight elevator. Purpose: the purpose of this research was to see the implementation of distribution of outpatient medical record, knowing the factors supporting the implementation of the distribution of medical record and knowing the percentage of medical record file speed up to the clinic. Research method: the type of this research is a descriptive qualitative case study design. The object of this research is the implementation of the distribution of medical record while the subject of this research is the official distribution, filing clerk, the clerk and Chief of Clinic medical records of installation. Data collection techniques are obtained by means of observation, interview and documentation study. Test the validity of the data using triangulation of sources and triangulation techniques. Results of research: the implementation of distribution of outpatient medical record begins with the patient registration on the registration patient site. Outpatient towards filing for her medical record file resolved the next distributed to such purpose. The factors supporting the distribution of medical record includes: behaviour of human resources, a good budget planning, implementation of activities based on SOP, any medical record and file tracer that uses a color code, there is a wide range of supporting tools and means of distribution. The percentage of the speed distribution of the medical record before and after the distribution using the tool using the tool of the overall distribution has increased, i.e. by June 2013 amounting 36,75% and in June 2014 risen to 47,59%. However, there is still a medical record that is distributed to the clinic more than 30 minutes.
