- Background: Medical Record is an important document of a patient that contains a history of the patient's pain. Thus requires a management and organization of medical records properly and in accordance with standard procedures (work procedures and work instructions). Management and delivery of medical records conducted by medical records personnel (medical recorder) or an administrative officer in the health service. In the case that the designer take on the issue of the absence of some work procedures and work instructions on the unit medical record that is causing some of the medical record officers doing their duties in accordance with their perception - each. At the health center II Gondokusuman Yogyakarta will also hold ISO 9001: 2008, which is a prerequisite certification is the work procedures and work instructions.
