  • Background: Hospital as a health institution shall provide a good public service. In order to provide a good public service, then performed the hospitals accreditation. One of the aspects that are surveyed in the hospital accreditation standard 2012 is closed medical record review about assessment and continued medical record review. There are standard group focus to the patient (APK, AP, PP, PAB, MPO, PPK) and standard group hospital management (MKI) in the format of medical record review. Based on hospital accreditation standard 2012, hospitals get a certificate of accreditation if each chapter of the hospital accreditation standards has a value of at least 80%. From the results of the study of documentation in RSUP Dr.Sardjito, average completeness of closed medical record review in January 2014 reach 65,80%. Objective: knowing the percentage of completeness closed medical record about assessment and continued medical record in RSUP Dr. Sardjito based on hospital accreditation standard 2012, knowing the factors that cause incomplete medical record about assessment and continued medical record and also knowing the efforts to resolve incomplete medical record about assessment and continued medical record. Methods: this type of research is a descriptive study with qualitative approach and case study research design. For collecting data techniques using interview, observation, and study documentation. For the validation of data using triangulation sources and triangulation techniques. The sample subject using purposive sampling. Sampling object in this research with probability sampling by random sampling. Object of this research is the inpatient medical record files that have been stored in December 2013 to March 2014, there are 98 samples file. Result: The average percentage of completeness closed medical record about assessment reaches 100%. The average percentage of completeness continued medical record reaches 84,19%. Factors that cause incomplete closed medical record are the flurry of doctor that cause inaccuracy filing medical record (man), there are too much item on the inpatient medical record form (material), SOP have not been socialized (method). Efforts have been made to overcome the incompleteness of filling are socialization, analyze the medical record, and communication among health care personnel.
