  • Background: Mlati 1 Sleman Yogyakarta health center has been using the software / management information system health center (SIMPUS), where this software provides various types of menus with their respective functions and one of them is \"menu reports\" that serves to create several types of reports, including monthly reports. But it has not been used by the health center in the making monthly reports so that the monthly report making is still done manually, therefore, important to know the factors that cause health center management information system (SIMPUS) not used in the making of the monthly report. Objective: To determine the causes (Man, Money, Materials, Machines, Methods) was not used in the making SIMPUS monthly report on Mlati 1 Community Health Center Sleman Yogyakarta Method: The type of research that will be used is a descriptive study with a qualitative approach Results: The monthly report is made in the health center in Sleman Yogyakrta Mlati 1 consists of morbidity reports (LB1), reports of drug use and the request (LPLPO), community service (LB3), visit health centers (LB4). Morbidity reporting process (LB1) using SIMPUS but do editing. LPLPO, LB3 and LB4 created manually. Factors causing SIMPUS disuse in making monthly reports is low motivation to do data entry officer outcomes of care provided to patients at the time of service. Differences morbidity report format (LB1) and reports of drug use and the request (LPLPO) to format a report of Sleman District Health Office. SIMPUS can make changes or additions to the data of drugs, but there is a slight discrepancy is the result of the addition of drug data will appear on the last serial number. In addition, there are standard operating procedures for the preparation of monthly reports using SIMPUS.
