- Background: epidemiological studies showed a trend of increased incidence and prevalence rate of diabetes mellitus ( DM ) in various parts of the world. The amount of the estimated number of patients suffering from diabetes mellitus is associated with increased micro- and macrovascular complications. Increased complications of diabetes occur due to vascular disorders that preceded the increased viscosity of the blood flowing. Number of endothelin B receptor is reduced due to vascular disorders. In vitro studies indicate shear stress can increase expression of ETB receptors on the endothelium. Shear stress increases in physical exercise. Physical exercise can improve ekepresi suspected ETB receptor, increasing the diameter of blood vessels, and prevents the increase in blood pressure in diabetes mellitus. Objective: This study aims to deepen the effect of regular physical exercise in a mouse model of DM to changes in blood pressure, knowing endothelin B receptor expression changes in the endothelium of the aorta, aortic diameter, and blood pressure -induced rat model of diabetes mellitus treated with streptozotocin after physical exercise routine. Methods: This study is a purely experimental research design with pre - post test control group for blood pressure measurement, and a post- test only control group for measuring the diameter of the aorta and endothelin B receptor expression in the aortic wall. Rat model of DM obtained by injecting streptozotocin 35mg/kgbb intra peritoneal Sprague Dawley male rats aged 11-12 weeks. Rats were divided into two groups, namely the rat model of diabetes mellitus who received regular physical exercise and group rat model of diabetes mellitus without regular physical exercise. Physical exercise routine using a special treadmill mice, low - moderate intensity, frequency 5x/week, for 9 weeks, with a week of adaptation using a speed of 5 m / min for 10 minutes, followed 8 weeks later using a gradual rise velocity of 5m/menit-20 m / min for 60 minutes. Blood pressure was measured from the tail vein in mice using a specific sphygmomanometer brands IITC. Rat aortic diameter was measured lumen diameter rat aortic preparations with hematoxylin eosin staining, were measured three times then taken using a microscope optilab reratanya. Endothelin B receptor expression was calculated by the percentage of the endothelin B receptor in aortic endothelial who has been stained with immunohistochemical staining with the formula number of positive stained endothelial cells: the total number of endothelial cells x 100 % . Results were analyzed by two groups of different test statistics, independent t - test. Results : 35mg/kgbb streptozotocin injection can increase blood sugar levels in diabetes group by more than 140mg/dl . Results of independent t - test for the expression of endothelin receptors in the DM group without diabetes with exercise and group exercise p value = 0.006, this indicates a significant difference between the two groups. Results of independent t - test test for aortic lumen diameter in diabetic rats with exercise group and DM group rats without exercise produces pvalue = 0.033, which means there is a significant difference between the two groups ( p < 0.05 ). While the test results of independent t - test for difference in systolic pressure in the two groups p value = 0.355. These results indicate that there is no statistically significant difference between the change in systolic blood pressure with exercise and DM group without exercise . Conclusion : Blood pressure rat model of streptozotocin -induced diabetes and treated physical exercise was not significantly different to the untreated group physical exercise . Endothelial endothelin B receptor expression of rat aorta model of streptozotocin -induced diabetes significantly more in the group receiving exercise treatment than the untreated group physical exercise . Aortic diameter rat model of streptozotocin -induced diabetes was significantly greater in the group receiving exercise treatment group than in the untreated physical exercise. There is a relationship between the expression of endothelin - B receptor in endothelial aortic systolic blood pressure, but there was no correlation between the diameter of the aortic blood pressure.
