- Warfarin is an oral anticoagulant which has very narrow therapeutic window. Monitoring of warfarin in patients is usually done by their pharmacodynamic effects prothrombin time (PT) with results expressed as INR. Monitoring of the plasma concentration of warfarin can help diagnosis and allows for the effective treatment of severe intoxication. This study aims to estimate the plasma concentration of warfarin, to know the clinical outcome seen from INR value, and to determine the correlation of warfarin plasma concentration with INR. This study was an observational retrospective study with descriptive and correlative design. The subject of this study was outpatients who have received oral warfarin. The estimated plasma concentration of warfarin was count based on pharmacocinetics equation, and correlation between warfarin concentration and INR was assessed by Spearman analysis. The mean estimated average plasma concentration of warfarin at steady state (Cp ) of 86 patients within 1 month of warfarin use was 0,658 ± 0,315 mg/L, mean minimum concentration at steady state (Cp ) was 0,135 ± 0,065 mg/L, and mean maximum concentration at steady state (Cp ) was 0,802 ± 0,384 mg/L. Forty four patients (62,79%) who have received warfarin dose 2,15 ± 0,74 mg per day, didn�t reach INR target (average INR was 1,29 ± 0,30, which range 0,90 � 1,93). Thirty two patients (37,21%) who have received warfarin dose 2,41 ± 0,80 mg per day, reached INR target (3,10 ± 0,93), which range 2,01 - 5,30. The correlation of warfarin plasma concentration with INR was not significantly different (p = 0,180), and the coefficients were 0,146 means that the correlation was very poor. The mean estimated Cp was 0,658 ± 0,315 mg/L at day 30. Forty four patients didn�t reach INR target and 32 patients reached INR target. The correlation of estimated warfarin plasma concentration with INR was very poor.
