- Backgrounds: The hospital is a health care institution that organizes personal health services in the plenary. One of the hospitals in an effort to provide good service to patients is by doing a good recording and documentation recorded in the medical record. Medical Recorder is obliged to make a written report on health care for the implementation of internal and external report generation. The report is a study of the structure and the information set forth in full, clear, comprehensive, correct, objective, and systematic. In this connection, the report can be used as a source for the consideration of a policy or decision-making for the work unit and the organization as a whole. Goals: This study aims to determine the internal reporting process in Unit Rekam Medis Rumah Sakit Bhayangkara Polda DIY in 2013 Methods: This type of research is a descriptive study a qualitative approach with a cross-sectional design. The technique of collecting data using interviews, observation, and documentation studies. Result: Internal report made in Unit Rekam Medis Rumah Sakit Bhayangkara Polda DIY in 2013 was in accordance with the requirements, the report consists of a report in the format X-KR and incidental reports. Reporting formats have not done X-KR socialization and evaluation of the use of the report to the parties and has not been done internally archiving for incidental reports in medical records. The process of making a report in Unit Rekam Medis Rumah Sakit Bhayangkara Polda DIY in 2013 consists of data collection, data processing, and data presentation. In the process of data collection and processing is done manually and is specialized in the implementation of data processing is less efficient.
