  • The orthodontic treatment with or without extraction can affect the facial profile. The changing on facial appearance is result of anterior teeth reposition. Changes on inclination and position of teeth were caused by teeth protraction or retraction, that can effect on the facial soft tissue profile changes directly, especially in the lips area. The orthodontic treatment planning with or without extraction is still debatable manner, especially in class I borderline cases. The aim of this study was to compare the changes of lips profile and incisors position in class I borderline cases treated with extraction of 4 second premolars and nonextraction. The study was conducted on 28 lateral cephalograms of class I borderline cases that inclusion the criteria, which were consist of 15 non-extraction cases and 13 cases of extraction 4 second premolars. Each cephalograms had measured on its lips profile and incisors position at pre and post-treatment. Measurement of lips profile consist of linier and anguler measurements, which were the distance of upper and lower lips to Vertical Reference Plane (VRP) and interlabial angle, whereas measurement of incisors position were the distance of upper and lower incisors to Vertical Reference Plane (VRP). The results of this study have showed both groups had similar characteristics on lips profile and incisors position (p>0,05) before the orthodontic treatment. Significant differences (p<0.05) regarding lips position, interlabial angle, and incisors position changes have occurred between the extraction and non-extraction groups after orthodontic treatment. Conclusion: lips profile and incisors position in class I borderline cases treated with extraction 4 second premolars have more retruded than the non-extraction cases.
