- Background: One of the household pollution indicators is exposure to smoke. Temanggung District which has cool weather allows people to do some activities that maintain the body warm such as making of a fireplace in the kitchen and smoking habits. The smoke resulted is an indicator of in-house air , especially when coupled with burning of mosquito coils inside the house. The research was conducted to examine the relationship of household pollution exposure as risk factors of pneumonia incidence in under five children. Methods: This research was an observational study with a matched case-control design and a ratio of 1:1. The total sample was 230 respondents, drawn from records of the register based on IMCI care of public health center (PHC). The case was children under five suffering from pneumonia that came to the PHC for treatment as a new diagnosed and classified pneumonia case by the physician/ health care officers and met the inclusion and exclusion criteria set. The control was children under five suffering from pneumonia that came to the PHC for treatment and were declared as pneumonia non-sufferers based on the diagnoses and classifications by the physician/health care officers and they met the inclusion and exclusion criteria set. The data were analyzed using X2 Mc Nemar test, OR and conditional logistic regression. Results: Most of the subjects of the research are male children under five and aged less than 12 months. After the other variables are adjusted, the household pollution exposure as a risk factor of pneumonia incidence of children under five in Temanggung i.e. the children who had high frequency to gather with family members who had the habit of smoking (OR=3,8
