- Background: Emergency unit of the General Hospital of Jayapura is the entrance for patients directly to the hospital and referral center in Papua. Because the hospital is located in an area away from the center of the state, the hospital depends on the context of local development. The hospital is located in an area with sparse population and spread over large areas. Due to the limited number of personnel, doctors at the hospital have a lot of tasks outside the hospital. As other government agencies in Indonesia, this hospital has a bureaucratic culture with a very weak supervision. Objective: This study intends to study the emergency room in response to maternal emergencies. This study is expected to provide recommendations in order to improve maternal care. Method: This research is a case study with qualitative data. Respondents consisted of the head of the hospital, nursing unit head, head of emergency unit, head of inpatient rooms, emergency room nurses, OBGYN doctors, head room, midwife Midwifery room, officials from the Red Cross, and the patient's family. Data were obtained through in-depth interviews, direct observation, and a review of documents recording and reporting. Results: Maternal cases referred or who come directly to the ER are sent straight to the maternity section. Midwife are rarely available in the emergency department because of the high burden in midwifery unit. Workers in the emergency unit has a low work motivation. They never got a refreshing courses related to their work. Team working in ER unit went bad because of the broken phone and no special operator to manage the communication with external parties. This unit works alone. Team work goes very weak. There is no official cooperation with partner institutions hospital. Coordination meetings are rarely performed. Conclusion: This study shows the case of maternal emergency services management at a government hospital with weak supervision. Emergency unit running with leadership dominance of physicians who have multiple jobs. This study confirms the existence of the managerial problems that seriously impact on patient safety in hospitals. Managers who are not full time weaken (1) coordination between hospitals, (2) reduce the motivation of health personnel work, and (3) undermine the planning and implementation of logistics systems and equipment. Suggestions: Hospital managers need to separate the tasks of clinical management and the management of the emergency unit. Senior midwives need to be involved in a more formal and larger in the management of the emergency unit. Thus, an experienced senior midwives gain a clearer status in regulating the employment of physicians involved in maternal emergency services. Hospitals need to strengthen the capacity of telephone communications in order to support the smooth coordination between hospitals.
