  • Background: To improve the quality of health services, hospitals must achieve an administrative order that is realized in the form of medical records management system. So that services and activities at the hospital should be run properly. At the Regional Mental Hospital (RSJD) Dr. RM. Soedjarwadi Klaten, still not fully perform the work in accordance with standard operating procedures. Outpatient registration, registration of inpatients, emergency room patients or registration has not been fixed in accordance with the implementation procedures. Process in the implementation of the procedure remain in the hospital should be implemented in accordance with standard operating procedures. At the Regional Mental Hospital Dr. RM. Soedjarwadi Klaten there are problems in the registration section of the patient remains less maximum use of existing procedures. Objective: This study aimed to determine the patient registration process, evaluate the implementation of standard operating procedures that exist in RSJD Dr. RM Soedjarwadi Klaten and determine implementation procedures remain patient enrollment, as well as determine the ratio between fixed procedure with patient registration service implementation. Methods: The study was a descriptive study with a qualitative approach with a cross-sectional study design. While data collection techniques with interviews, observation, and Study documentatio n. Results: The registration of outpatient, inpatient and emergency department in accordance with standard operating procedures that exist in RSJD Dr. RM Soedjarwadi Klaten. Children in particular clinic, the implementation of long patient outpatient services not in accordance with standard operating procedures are applied. In addition, the manufacture of tracer in RSJD Dr. RM Soedjarwadi Klaten inconsistent and not in accordance with the procedure because some tracer made by the registrar and partially created by the filing officer. Suitability for the percentage of enrollment of new patients is still low, registering 48.48% outpatient, inpatient registration 42.86%, and 65.15% emergency registration. As for the percentage of time patients remained low in the outpatient registration is 43.75%, 35.71% enrollment hospitalization, and emergency room registration 50.00%.
