  • The prevalence of infectious diseases quite high despite treatment and preventive therapy on the incidence of infection is growing. Audit the use of antibiotics both qualitatively and quantitatively is one way that can be done so that antibiotic prescribing rationaly. The purpose of this reseach were to determine the pattern of empirical therapy, audit quantitative and qualitative use of antibiotics as both empirical and definitive therapy and determine inter-reviewer reliability in assessing the quality prescribing of antibiotic rationality. This research was a descriptive study using cross-sectional design with prospectively data collection on patients diagnosed infection who meet the inclusion criteria and hospitalized in internal medicine department Dr. Sardjito during the period December 2011 to February 2012. Assessment the quality of antibiotic based on Gyssens algorithm conducted by three reviewers consist of two internist and a pharmacist. Assessment the quantity of antibiotic is done by comparing the value of Prescribed Daily Dose (PDD) with Defined Daily Dose (DDD) that issued by WHO Collaborating Centre for Statistics Methodology in 2011. Inter-reviewer reliability in assessing the rationality of antibiotic performed statistical analysis using Cohen's kappa with Medical Calculation (MedCalc) 12nd version program. The results showed total population of 1.042 patients, 68 patients who met the inclusion criteria. Most empirical antibiotics used were ceftriaxone 45 cases (66,18%). Audit qualitatively by three reviewer agreement 30 cases rational and 27 cases irrational in empirical therapy. Otherwise 46 cases rational and 19 cases irrational in definitive therapy. Audit quantitatively of antibiotics prescribing showed ceftriaxone and metronidazole have an equal value to the DDD and PDD. Results of the analysis showed good reliability with kappa coefficients between 0,61 - 0,81.
