- The background of this research is the enactment of regional autonomy and decentralization of health and malaria, should be the granting authority to the government will improve community health status, but the reality of the discovery of field numbers and health numbers of malaria cases is still high. The research objective is to reveal how the implementation of cost management the malaria eradication program. Information and research data derived from, the head of the health department, district, hospitals, and clinics that have relevance to the focus of this research. This type of research is qualitative research with study sites in the health districts of Biak Numfor. Data base collected through observation, interviews and documentation. Validity of data is done through, triangulation, methods and data sources. The presentation of the data analyzed by data reduction and inferences. The results showed that the Biak Numfor has implemented a program to eradicate malaria with the cost management approach to fund the program, but finding the numbers of cases and morbidity due to malaria is still high and most of the malaria program is not implemented because of funding delays. Advice, malaria should be eradicated through the optimization of resources seriously while adequate financing of health resources, the method means, for lack of seriousness will be fatal for the local community.
