  • Previous studies showed significant increased in the number of out-patient with type 2 diabetes, especially in the primary health care center area of the Province of Yogyakarta, Indonesia. Poor self-management of patient with type 2 diabetes lead to complication resulting in death. This study examines the effectiveness of training an active patients in self-management diabetes to decrease blood sugar levels, increase illness perceptions, decrease diabetes distress, increase self-efficacy and increase self-management behavior adults with type 2 diabetes. The training's module was developed from the Common-Sense Model. The subjects were patients with type 2 diabetes from Gamping II primary health care center (10 patients to experiment group and 10 patients to control group). Data were collected through pretest, posttest, and follow-up. The Mann-Whitney test showed that blood sugar test wasnâ��t significant (Z=-1.626
