- Background: Muscle relaxant on RSII using succinylcholine has an advantage of good intubation condition in 60 seconds, regarding of its limited availability and side effect on this regiment, other alternatives should be considered to find. Rocuronium has rapid onset and probably could serve as an alternative muscle relaxant on RSII. Atracurium usage as priming dose againts rocuronium intubation dose hopefully would produce such faster onset without block duration prolongation. Objectives : To compare the condition of larynx before intubation at 45th seconds after rocuronium 0.6 mg/kgBW iv with or without priming dose atracurium 0.12 mg/kgBW iv. Methods: This research was designed as a double blind randomized clinical trial. Subjects were 88 patients who underwent elective surgery with endotracheal intubation general anesthesia at GBST Dr. Sardjito Hospital Yogyakarta and met the inclusion criteria, male or female, age 18-60 years and physical status of ASA I and II. They were divided into two groups amounted equally. Group A received atracurium 0.12 mg/kgBW iv and group B NaCl 0,9 % 1,5 cc iv. All subjects received induction propofol 1.5 mg/kgBW, and rocuronium 0.6 mg/kgBW 3 minute after priming dose. After 45 seconds, larynx condition was observed using Kreig�s score and we recorded the TOF value. Intubation was performed. Results : The larynx condition at intubation after 45 seconds rocuronium 0.6 mg/kgBW showed 97.7 % 'excellent' score and 2.3 % 'good' score in grup A and grup B 84.1 % 'excellent' score and 15.9 % 'good' score. It was statistically significant (p = 0,026). Conclusion: Priming dose atracurium 0.12 mg/kgBW iv followed by rocuronium 0.6 mg/kgBW iv provides better larynx condition while intubation at 45th secondsthan rocuronium 0.6 mg/kgBW iv without priming dose atracurium 0.12 mg/kgBWiv (p = 0,026).
