- Background: The global objective to improve infant�s health and food quality optimally is with exclusive breastfeeding from birth to 6 months of age. IDHS data indicate that exclusive breastfeeding in infants less than 6 months has dropped to 32% in Indonesia and 38% in the whole world with a median duration of breastfeeding 22 months and in 2007 the median was 23.7 months. The use of contraceptives increased from 50% in 1991 to 61% in 2007 in which the change in use of modern contraceptive methods of KB occurred. Use of contraceptive pills and injectables is increasing. Contraception used during breastfeeding should be a safe contraceptive method, but combined hormonal contraceptive methods especially those containing estrogen and progesterone may interfere with lactation by inhibiting prolactin thereby reducing milk production. Objective: To obtain a description about the effects of hormonal and non hormonal contraceptives on breastfeeding duration in Indonesia. Methods: This was an observational study with a retrospective cohort design study using 2007 IDHS data in. Study samples were women aged 15-49 years who met the inclusion and exclusion criteria that totaled 10738 in 2007 IDHS using pills,injectable/implant and non-hormonal contraception as the independent variable and duration of breastfeeding as the dependent variable. Analysis of data used survival analysis, where the influence of hormonal and non-hormonal contraceptive use against duration of breastfeeding was obtained using Cox proportional hazard model. Results: Based on the 2007 Indonesia Demographic and Health Survey, the use of contraceptive pills was not significant on duration of breastfeeding HR = 0.99 (95% CI=0.89 to 1.12) and injectable / implant had no significant effect on breastfeeding duration with HR = 0.90 (95% CI=0.82 to 0.99). The duration of contraceptive use at 0-6 months was significant to the duration of breastfeeding with HR = 2.86 (95% CI=2.20 to 3.31). In addition, the duration of contraceptive use at 7-12 months was significant to the duration of breastfeeding with HR = 1.77 (95% CI=1.32 to 2.28). Conclusion: The duration of breastfeeding using contraceptive pills was faster to wean than non hormonal and injectable / implant and the duration of contraceptive use was more rapid to wean at 0-6 months compared with 7-12 months and 13-24 months.
