- Background: Motivation is a basic impulse that drives a person to act to perform the activity or activities that are expected. The motivation to retain permanent teeth due to caries reflected one of the performed treatment index (PTI) is calculated from the number of permanent teeth are patched divided by the total value of DMF-T. PTI value West Sumatra province is 0.07%, lower than the national average value (1.6%). This study aims to determine the factors that influence the motivation to retain permanent teeth due to caries in the community who visited community health center dental clinic of Padang Municipality in West Sumatra Province. Methods: This study is a survey research using a cross sectional approach, with the total subject 150 people. Questionnaire were used to asses the dependent and independent variabel and have tested validity (correlation values � 0.30) and reliability (α > 0.60). The questionnaire is required to measure knowledge, perceptions, attitudes, facilities and infrastructure, age, gender, education level and dental caries status examination form is to find the motivation to retain permanent teeth due to caries in the community who visited community health center dental clinics. Data is analysed by : univariate, correlation, regression. Result: Gender and education level had no significant effect on motivation (p> 0.05), multiple regression analysis showed that two independent variables
