  • Background: One of the important issues in the development of national health is the limited accessibility of the health services. This problem also occurred in Lingga District, Riau Archipelago Province. This is caused by variety things, such as geographical location, cost, number of health workers and also the facility of health service, including primary health centre and its networks that are not fully accessible to the public. Several attempts have been made, for example by improving the status of supporting primary health centre to primary health centre , primary health centre to caring primary health centre, putting the health workers both medical and paramedical, increasing the health financing budget and create a supportive policy. In the other hand, the implementation has not been fully implemented. In accordance of the background above, required a study on the role of the local government policy in improving the referral system that is useful to identify problems in the field, so the various repairs could be done in the future. Objectives: To know how the referral system in the islands territory in Lingga District. Methods:. This study is a descriptive study by using case study design. The subjects are the head of primary health centre / physician in primary health centre , nurse/midwife partner, ambulance driver, shipmaster of clinics mobile boat, patient family, public figure, community health insurance/district health insurance manager, head of the Lingga District health centre/head of the local health department, the director of the Lingga Government Hospital / Lingga Field Hospital, and the nurse of Emergency unit in hospital. There are 2 variables in this study, they are independent variable (referral system) and dependent variable (ambulance service). The location of this study is Lingga District, Riau Archipelago Province. Results: The result of the research showed that there are already some policy efforts from the government of Lingga District in improving the referral system. The existing policy of financing has covered two aspects both in terms of demand (medical expenses) and in terms of supply (system that supports the health service). The referral process from primary care to secondary care went well yet there is still a lack of attention to the aspects of availability and completeness of the service type. Most of the health workers had received training, specialists are available (in collaboration with the faculty of medicine), but the networking in the referral process still done partially and not fully integrated. Conclusion: The health referral system in Lingga District has been wellimplemented, but not fully involving the participation of the society in an integrated service system. The government in this case, the Lingga District Health Centre needs to revitalize the perceptive village as well as accelerating the development to increasing the participation of the society in developing the referral system.
