- Medical record information system is a system that regulates the file that contains records and documents about the identity of the patient, examination, treatment, action, and other services provided to patients. The problems that occur in home care psycho - neuro - geriatric "Puri Saras" is absence of a system that regulates medical record file. Expected with the information system is a web-based medical record can assist officers in managing patient medical records and minimize the loss of patient data that can later be fatal to the health of patients and further examination.Other than that to the web-based information can be accessed with a time and place not specified. In this system menu can only be accessed by certain users doctors, staff, and administrators. This system will be developed by the method of the System Development Life Cycle (SDLC), Entity Relationship Diagram (ERD), as well as using the PHP programming language and MySQL as the database.
