- In the organization of medical records is divided into three, namely registration, storage and processing of medical records. Registration is among the systems of medical records implementation, within the existing system of registration details registration system, the system of naming, numbering systems, system KIUP (Main Index Card Patient). Every patient who comes to health care institutions by medical record number that serves as one of them the identity of the patient. Each patient only get one medical record number that is used both for outpatient and inpatient (Unit Numbering System), one beam is supervised by a single patient medical record number. Interest numbering medical record is to distinguish the medical records of patients with each other. Numbering duplication that occurs mainly due to lack of proper identification process that would cause a patient to get more than one medical record number. To determine the factors that influence the numbering duplication of medical records at Atma Jaya Hospital. This study used qualitative research, conducted interviews with admissions officers. Analysis of data using interview, observation and primary data. Duplication numbering at registration didapetin patients where patients get the doubles, and every day about 1-4 patients who get the doubles. Educational qualifications, knowledge, and experience less conscientious and less aware of the numbering system of medical records. Still numbering duplication of medical records and personnel necessary for the training and increase insight. It is expected that the hospital could not give attention to the registrar in the numbering of the medical record.
