- Background. The discovery of case and early treatment of hypothyroidism in under three years children is very important because late diagnosis will cause permanent intellectual and neurological dysfunction. Hypothyroidism can be diagnosed more accurately by laboratory examination, but it will be expensive and less practical when conducted in the field setting. Hypothyroid screening instrument tools (ISH) has been used as an alternative for the detection of hypothyroidism in children among under three years. The instrument should be easy, cheap and can provide a reliable diagnostic result. Objective. The aim of this study was to assess the sensitivity and specificity of the hypothyroid screening instrument for diagnosing hypothyroidism. Method. This study was conducted in Magelang, Wonosobo, Temanggung, Purworejo, Situbondo, and Jember, for ten months. Twenty-six symptoms of hypothyroid were analyzed using crossed tabulation with 5% significance level. Results. The sensitivity and specificity of the hypothyroid screening instrument (ISH) compared with the gold standard is low (Se 26.3, Sp 90.3, area under curve (AUC) 58.3%). The strength of the relationship hypothyroid screening instrument (ISH) compared with the gold standard is OR 3.329 (1.621–6.835; 95% CI). The multivariate test corrects the probability of a toddler who has an ISH score of >5 to become hypothyroid 2.253 (1.011-5.022, 95% CI), area under curve (AUC) increase to 70%. Conclusion. Hypothyroid screening instrument in children among under three years was neither sensitive nor specific for diagnosing hypothyroidism.
