- [<b>ABSTRAK</b><br> Praktek Kerja Profesi Apoteker di Rumah Sakit Umum Pusat Fatmawati bertujuan untuk mengetahui gambaran umum RSUP Fatmawati, mengetahui struktur dan pembagian kerja di Instalasi Farmasi RSUP Fatmawati, dan mengetahui peran dan tanggung jawab apoteker dalam Peran Lintas Farmasi. Tujuan khusus berjudul Pemantauan Terapi Obat Pasien Ulkus DM dengan Gagal Ginjal Kronis di Lantai V Utara Teratai RSUP Fatmawati agar mahasiswa calon Apoteker dapat mengidentifikasi masalah yang berkaitan dengan penggunaan obat pada pasien ulkus DM dengan gagal ginjal kronik Lantai V Utara Teratai dan memberikan rekomendasi intervensi untuk masalah yang berkaitan dengan penggunaan obat yang dapat terjadi. <hr> <b>ABSTRACT</b><br> Apothecary Internship at Rumah Sakit Umum Pusat Fatmawati aimed to determine a general overview RSUP Fatmawati, determine the structure and division of labor in the Instalasi Farmasi RSUP Fatmawati, and determine the roles and responsibilities of Apothecary in the role of Cross Pharmacy.
