Penelitian lainnya menyatakan bahwa penambahan bupivakain hiperbarik segera setelah fentanil intratekal dapat meningkatkan durasi analgesia. Kami membandingkan penambahan bupivakain 2,5 mg hiperbarik dengan penambahan klonidin 45 μg pada campuran bupivakain 2,5 mg 0,1 % dan fentanil 25 μg.
Metode : Sebanyak 34 ibu bersalin dengan pembukaan serviks z3 sm dirandomisasi ke dalam 2 grup. Grup pertama menerima campuran bupivakain 2,5 mg 0,1% dan fentanil 25 pg diikuti oleh bupivakain 2,5 mg 0,5% hiperbarik (grup BH, n=17). Grup kontrol menerima campuran bupivakain 2,5 mg 0,1% plus fentanil 25 μg plus klonidin 45 μg diikuti oleh NaCl 0,9% (grup K, n=17). Dari posisi duduk saat penyuntikan obat, pasien dibaringkan dengan posisi 30° sepanjang sisa persalinan. Dilakukan pencatatan durasi analgesia, skor VAPS, skor Bromage, efek samping, lama persalinan, jenis persalinan, skor APGAR dan tingkat kepuasan ibu bersalin.
Hasil : Ibu bersalin dalam grup K memiliki durasi analgesia yang lebih lama (Tara-rata 168 menit, kisaran 140-240 menit) daripada ibu bersalin dalam grup BH (rata-rata 126 menit, kisaran 105-150 menit) (p<0,001). Grup BH juga memiliki lebih banyak ibu bersalin yang mengalami blok motorik (p=0,003). Efek samping lainnya seimbang di antara kedua grup, dengan catatan bahwa tidak ada kejadian mual-muntah pada kedua grup, dan hanya 1 kejadian hipotensi pada grip BH.
Kesimpulan : Penambahan klonidin 45 μg pada bupivakain 2,5 mg 0,1% dan fentanil 25 μg menghasilkan durasi analgesia yang lebih lama dibandingkan penambahan bupivakain 2,5 mg 0,5% hiperbarik segera setelah bupivakain 2,5 mg 0,1% dan fentanil 25 μg.
<hr><i>Background : One study reported that the addition of clonidine 45 pg to intrathecal bupivacaine and fentanyl reduced blood pressure and did not increase the duration of analgesia. In another study, the addition of hyperbaric bupivacaine right after intrathecal fentanyl increased the duration of analgesia. We compared the duration of analgesia of intrathecal hyperbaric bupivacaine 2,5 mg right after intrathecal bupivacaine 2,5 mg 0,1 % and fentanyl 25 μg with that of intrathecal bupivacaine 2,5 mg 0,1% and fentanyl 25 μg and clonidine 45 μg.
Method : Thirty-four parturients with a cervical dilation 3 cm were randomized into 2 groups. The first group received intrathecal bupivacaine 2,5 mg 0,1% and fentanyl 25 μg which followed immediately by hyperbaric bupivacaine 2,5 mg 0,5% (group BH, n=17). The second group received intrathecal bupivacaine 2,5 mg 0,1% and fentanyl 25 μg and clonidine 45 μg followed immediately by NaCl 0,9% 0,5 ml (group K, n=17). After the administration of the drugs, the position of the patient was changed from sifting to supine with 30° elevation of torso. We collected the data of duration of analgesia, VAPS score, Bromage score, other side effect, duration of labor, type of labor, APGAR score and the maternal satisfaction.
Results : The duration of analgesia of group K (mean 168 minutes, range 140-240 minutes) is longer than group BH (mean 126 minutes, range 105-150 minutes) (p<0,001). There was more patient with motoric block in group BH than in group K (p=0,003). The other side effects are equal ini both groups. We noted that there was no nauseaNomiting in both group, and there was only one patient BH got hypotension which treated easily.
Conclusion : The addition of clonidine 45 μg to intrathecal bupivacaine 2,5 mg 0,1% and fentanyl 25 pg results in longer duration of analgesia compared with the addition of hyperbaric bupivacaine 2,5 mg 0,5% right after bupivacaine 2,5 mg 0,1% and fentanyl 25 μg.</i></description><identifier>http://lontar.ui.ac.id/detail?id=107361</identifier><recordID>107361</recordID></dc>
