- The article shows that the «standard» protocols of cognitive-behavioral psychotherapy used in the treatment of disorders of intestinal-brain interaction in the form of irritable bowel syndrome (IBS) focus only on reducing the patient's reactivity to stress events of everyday life, while exposure therapy with the prevention of avoiding behavior (exposure with response prevention therapy) due to the impact on visceral sensations focuses on 1) elimination of maladaptive cognitive-affective processes involved in interoception and distorting the perception of bodily sensations; 2) the spectrum of disease-oriented search behavior. An interoceptive model of gastro-specific anxiety. The clinical example shows the specifics of the use of exposure therapy in the treatment of a patient with abdominal pain and diarrheal type of refractory IBS. The exposition is described in detail aimed at avoiding behavior, gastrointestinal anxiety, and reduced tolerance to uncertainty. Methods for solving dilemmas during exposure therapy are described. The components of a comprehensive assessment of the mental state of a patient with IBS are shown.
