- The overarching objective of the Work Package 9 (WP9) is to define a consistent methodology to incorporate fiscal impact in economic evaluations, focusing on productivity gains, consumption increases and potential tax revenues, deriving from health gains of new healthcare interventions. The investigation of the fiscal impact of a technology implies the adoption of a ‘government perspective’ investigating how resulting changes in morbidity and mortality associated with investments in health care technologies can influence national accounts both in terms of expenditure (i.e., transfers) and tax revenues. Such effects, which are usually overlooked in the traditional cost-effectiveness framework are of significant importance, when the perspective of the analysis is that of the ‘government’, for which the provision of allowances as well as the tax revenues forgone in case of ill-health represent a real value. This is particularly relevant for technologies with a strong impact on labor force participation, medical disability, early mortality, or retirement decisions.
