- PT PJB UPHT Gresik require a governance of information technology (IT) to develop a comprehensive framework of structure, processes, and every business processes completion method. Assessment of IT governance on Information Capital Readiness (ICR) that has been done by a team of assessors has a value of 3,5. Feedback report documents that are given by the assessor does not describe any remedial measures in achieving the desired target that is given by the central office, so PT PJB UPHT Gresik has a problem. Based on that problem, the design of IT governance in the ICR service operation refers to the assessment of performance contracting unit guidelines and service operation in ITIL V3 has been created. At the design stage, the gap analysis results that was obtained from performance contracting unit handbook and OFI contained in feedback report documents is analyzed in order to obtain an improvement identification. The results of the identification analysis are in the form of Standard Operation System (POB) and Working Instructution (IK), which both of them refers to the service operation process in ITIL V3 within the ICR. IT service operation governance design on the ICR produces two policies, the three POB (Infrastructure, Business Process Management and Information Management), and six IK (two work instructions on Infrastructure, three work instructions on Business Process Management and one work instruction on Information Management) that the company needs to make improvement on the ICR's performance process indicator.
