- Based on provincial health departement riau islands in 2019, IMR in the city batam recorded 30 /100.000 live births in 2017 to 25 /100.000 live births in 2018. While, in 2017 imr 162 /100.000 live births and happening a little decreased, namely 145 /100.000 live births. The method used by the author is by interviewing observations and providing on going care and management. To Mrs. T age 28 years old G4P2A1 for 7 weeks at BPM Hj. Nurkhairiyah Amd.Keb, SKM, M.H using the SOAP method. Continuous midwifery care (continuity of care) that has been carried out on Ny.T during pregnancy, childbirth, childbirth, newborns and family planning obtained examination results within normal limits and no complications espousing. It is expected that the midwife profession in providing continuous midwifery care (Continuity of Care) will always implement midwifery management, increasing competence in providing care according to midwifery service standards.
