- In providing information, Puskesmas Pembina Palembang do not have an application or information system. Patients and the surrounding society still should be coming directly to the Puskesmas Pembina Palembang to seek and to get information. This it has not efficient. Hence, with technology development currently, writer feel the need to build a information systems that can be provide information about Puskesmas Pembina and provide health services to the community as information about doctor at Puskesmas Pembina, various available poly, the activities to be performed in puskesmas, integration on the issue of health services and information of patients medical records. Writer aims to get a information system health services that are useful for all parties, especially for nurse poly and nurses part counter. Results obtained from research it will be information system health services in central the community health Pembina Seberang Ulu I Sub-District Palembang using PHP as programming language and MySQL as database.
