- Hospital as health care facilities are required health services to the community as much as possible. One effort to improve the quality of service is the administrative enforcement, including administrative control of medical record. Medical record itself is an activity recording patient information, anamnesis, physical determination of laboratories, diagnostic services, and all medical actions and treatments given to patients either hospitalized and outpatient. Without supported refineries medical record is good and right, then the orderly administration will not succeed. This research aims to design and build medical record application in RSIA Tiara Fatrin Palembang that can provide information about the patient’s healthy history. Stages of development of the research is using RUP (Rational Unified Process) with application Visual Studio 2012 and SQL Server as the database. This system is based on client-server. It can manage the data collection system, data collection of patients, outpatient and hospitalization, medical record and reports. The reports produce outpatients and inpatient. System testing technique using black box testing to check for errors in the system. Key word: Medical Record, Patient, Services
