- The purpose of this report is to make a registration application that can help the performance of the registration unit at Puskesmas Merdek Palembang by using the PHP programming language and MySQL database. The author finds some problems faced by Puskesmas Merdeka Palembang is still a manual processing enrollment using Microsoft Excel, the absence of information that can be shared to the community about the schedule and activities in an Puskesmas Merdeka Palembang, also the absence of a report that will be given to the Chairman of Puskesmas Merdeka Palembang regarding the number of patients who sign up every day, most diseases suffered by patients, and so on. Also by the existence of the report the number of patients who are enrolled based on poly Ministry that will be given to the head of the Puskesmas.
