The result of this research were obtained the highest damage index value was at cluster plot one of 3.32 and the lowest was at cluster plot eight of 0.04. The highest value of crown condition was at cluster plot eight of 3.91 and the lowest was at second cluster plots of 3.10. Forest health condition at plant and/or animal collection block had categorized by: 25% block area was very good, 38%block area was good,12% block area was medium and 25% block area was very bad.
