- Background : Grhasia is type A special hospital and serves as referral hospital of provincial level particularly in the mental health services, so it is required to provide professional and qualified service. One important factor in providing qualified services is its assured the availability of medicine, it always available when needed in sufficient quantity and quality is asssured. Formulary is one means to improve the efficiency and effectiveness of medicine management. The use of formulary as the therapy protocols can ensure qualifed prescription and cost effective. Formulary update is performed in each fiscal year, but still found problem in the availability of medicines, such as expired date of medicines, over stock of medicines and stock out. Objective : To know application of formulary, the availability of psychotic drugs and to identify the constraints in formularies application. Method : This research is descriptive with a case study research design. Data collection is carried out through observation, interviews and questionnaires. Result : Application of formulary in the planning stages psychotic drugs in 2011 do not fully. The conformity percentage of available psychotic drug items with formulary at 86,21%, percentage of damaged/expired psychotic drug items at 41,38% and over stock value of psychotic drug items at 7,43%. Psychotic drug items not found in a state of dead stock and percentage of stock out time average can not be calculated. Prescription performed by the mental health specialists to hospitalized psychiatric patients in 2011 was 100% refers to formulary. The reciped psychotic drug as much as 36 items (17 psychotic drug items with the generic name and 19 psychotic drug items with the trade name). All the mental health specialist have a good perception to formulary. Conclusion : Application of Grhasia formulary 2011 in the planning stages psychotic drugs do not fully but prescribing psychiatric patients refer to formulary. The availability of psychotic drug is not good because it was found that psychotic drug items damaged/expired and over stock. There are no constraints in application of formulary. All the mental health specialist have a good perception to formulary and prescription is an accordance with the formulary.
